Hello everyone
I am a French/English medical translator, and looking for hospital forms, to practice translating.
Of course I know I would want access only to blank forms, as otherwise confidentiality issues are invovled.
I have done multiple searches using "rapports hôpitaux" and "fiches hôpitaux" as well as "dossiers des patients" to no avail.
Any suggestions are appreciated.

I don't think you will find many of these documents on the internet because a) most hospitals have special websites for 'insiders' that are only accessible with a user name and password, and b) many hospitals are phasing out written records and replacing them with electronic systems.

While these are not "hospital records", the samples may be quite useful if you just want to familiarize yourself with the wording and structure of medical reports.

The site is exactly what I was looking for.
As a registered nurse in New York, I have seen thousands of hospital forms, but I wanted to see some in French to familiarize myself with the terminology.

I did some French-Thai hospital record translations but they were personal information. I guess that no one can disclose on Internet without permission of the specific patients. In addition, many handwritten parts are too poor in quality to read [and translate] --- very unclear characters that risk translation mistakes.
