Source text - English The objective of the research program is to identify the bio-psychosocial variations which seem to influence the clinical progress of heart transplant patients. Medical and clinical-psychological evaluation is longitudinal and integrates the rate of surviving heart surgery (death, re-transplant, chronic rejection, infection) with psychosocial variations.
Heart transplants have become a valid treatment for end-stage heart failure. The assessment of psychosocial aspects appears to be essential for the evaluation of the long-term outcome or patients. Psychological post-transplant complications can be distinguished into two groups. The first one includes Organic Mental Disorders which tend to occur in the immediate post-surgical period and are usually caused by immunosuppressive drugs or additional metabolic and/or psychological factors. The second group of psychiatric complications is represented by Mood Disorders and is often induced in the immediate post-operative period by the administration of corticosteroids.
Another frequent psychological reaction which is encountered during the post transplantation period is anxiety, often related to the fear of rejection, of infection etc. The incidence of mood and anxiety disorders in the heart transplanted population is high and has been studied by many authors. Therefore there is the need of a psychological evaluation that is able to take into consideration subclinical levels of psychological distress which can affect patient’s quality of life and compliance. The DCPR (diagnostic criteria for psychosomatic research) elaborated by an international group of researchers who authorized a more accurate psychological evaluation of the patient aimed at improving the patient’s quality of life.
Translation - Italian La finalità del programma di ricerca è l’identificazione delle variazioni biopsicosociali che risultano influenzare l’andamento clinico dei pazienti con trapianto cardiaco. Si tratta di un’analisi medica, clinica e psicologica longitudinale, che integra la percentuale di sopravvivenza all’intervento cardiaco (decesso, nuovo trapianto, rigetto cronico, infezione) a variazioni psicosociali.
Il trapianto di cuore è diventato una valida terapia per l’insufficienza cardiaca typo. L’analisi degli aspetti psicosociali risulta indispensabile per valutare l’esito o i pazienti a lungo termine.
Le complicanze psicologiche post-trapianto si possono distinguere in due gruppi. Il primo gruppo include i Disturbi Mentali Organici, che tendono a mostrarsi nell’immediato periodo post-operatorio, e sono normalmente causati da farmaci immunosoppressivi o da fattori aggiuntivi di tipo metabolico e/o psicologico. Il secondo gruppo di complicanze psichiatriche è rappresentato dai Disturbi dell’Umore, spesso indotti nel periodo post-operatorio dalla somministrazione di corticosteroidi.
Un’ulteriore reazione psicologica frequente riscontrata durante il periodo post trapianto è l’ansia, spesso legata alla paura di rigetto, infezione, ecc. L’incidenza di disturbi dell’umore e dell’ansia nella popolazione con trapianto cardiaco è alta ed è studiata da numerosi autori.
Di conseguenza, si pone la necessità di formulare un’analisi psicologica che sia in grado di considerare i livelli subclinici di stress psicologico che possono incidere sulla qualità di vita e sull’adesione del paziente alla terapia proposta.
Il DCPR (Criteri Diagnostici per la Ricerca Psicosomatica) elaborati da un gruppo internazionale di ricercatori, che ha autorizzata un’analisi psicologica più accurata del paziente, hanno lo scopo di migliorare la qualità di vita dello stesso.
