The examination was performed with sections 5 mm thick before and after the intravenous administration of contrast medium and in comparison to the examination of 08/02/2010.
History of patient undergone the surgery: Ca of left kidney on 12/2009 last time due to local recurrence. Follow up.
No pathological uptake of the contrast medium was observed or other type of focal lesion at the hepatic parenchyma. Cholelithiasis is retested without dilatation of the biliary tree. Adiposal degeneration of the pancreas. The spleen has physiological morphology and size free of focal lesions. No space-occupying lesion is observed at the adrenal glands. The contour of the right kidney is normal, its size and cortex thickness are normal. No radio-shadowy lapis or compact lesion are observed at the right kidney. Cortical cysts of the right kidney remain unchanged. The nephrectomy on the left is retested, without showing a mass of the soft molecules at the area of the surgical field. Nodulous amplified focal lesions around the lower pole of the spleen and the left posterolateral celiac wall at the area of the surgical field have increased in size in comparison to the previous imaging examination and are very suspicious of local recurrence. Indicative, one of these at the celiac wall at the level of the surgical field has a maximum anterolateral diameter at a transverse level of 16 mm versus a few millimeters at the previous imaging examination. No pathological intumescent paraaortic, pelvic, or bubonic lymph nodes are observed. A cystic lesion at the root of the mesentery remains unchanged. No ascitic collection is observed. Incomplete load of the bladder impedes the examination of its wall. The prostate gland and the spermatic cystidia are depicted normal. From the rough examination of the intestinal tube no apparent ectophytic type lesion was observed. The hernia of the white line is retested without apparent signs of furrowing.
Conclusion
Increase of the dimensions of the nodulously amplified condensive foci at the posterior celiac wall on the left, at the level of the nephrectomy and around the lower pole of the spleen are suspicious of local recurrence.

Η εξέταση πραγματοποιήθηκε με τομές πάχους 5 mm πριν και μετά την ενδοφλέβια χορήγηση του σκιαγραφικού μέσου και σε σύγκριση με την εξέταση στις 08/02/2010.
Ιστορικό του ασθενή που υποβάλλεται στη χειρουργική επέμβαση: Καρκίνος αριστερού νεφρού στις 12/2009 τελευταία φορά, που οφείλεται σε τοπική υποτροπή. Παρακολούθηση.
Δεν παρατηρήθηκε καμία παθολογική πρόσληψη του σκιαγραφικού μέσου ή άλλος τύπος εστιακής βλάβης στο ηπατικό παρέγχυμα. Η χολολιθίαση επανελέγχεται χωρίς διάταση του χοληφόρου δέντρου. Λιπώδης διήθηση παγκρέατος. Ο σπλήνας παρουσιάζει φυσιολογική μορφολογία και μέγεθος χωρίς εστιακές βλάβες. Δεν παρατηρείται χωροκατακτητική βλάβη στα επινεφρίδια. Το σχήμα του δεξιού νεφρού είναι κανονικό, το μέγεθος και το πάχος του φλοιού του είναι φυσιολογικά. Δεν παρατηρείται ακτινοσκιερός λίθος ή συμπαγής βλάβη στο δεξιό νεφρό. Οι φλοιώδεις κύστεις του δεξιού νεφρού παραμένουν αμετάβλητες. Επανελέγχεται η νεφρεκτομή αριστερά, χωρίς την παρουσία μάζας μαλακών μορίων στην περιοχή του χειρουργικού πεδίου. Οι οζώδεις ενισχυμένες εστιακές βλάβες γύρω από τον κάτω πόλο του σπλήνα και το αριστερό οπισθοπλάγιο κοιλιακό τοίχωμα στην περιοχή του χειρουργικού πεδίου έχουν αυξηθεί σε μέγεθος συγκριτικά με την προηγούμενη απεικονιστική εξέταση και είναι πολύ ύποπτα για τοπική υποτροπή. Ενδεικτικά, μια βλάβη στο κοιλιακό τοίχωμα στο επίπεδο του χειρουργικού πεδίου έχει μέγιστη προσθιοπλάγια διάμετρο 16 mm σε εγκάρσιο επίπεδο έναντι λίγων χιλιοστών κατά την προηγούμενη απεικονιστική εξέταση. Δεν παρατηρούνται παθολογικά διογκωμένοι παραορτικοί, πυελικοί ή βουβωνικοί λεμφαδένες. Μια κυστική βλάβη στη ρίζα του μεσεντερίου παραμένει αμετάβλητη. Δεν παρατηρείται ασκητική συλλογή. Η ελλιπής πλήρωση της κύστης εμποδίζει την εξέταση του τοιχώματός της. Ο προστατικός αδένας και τα σπερματικά κυστίδια απεικονίζονται φυσιολογικά. Από την αδρή εξέταση του εντερικού σωλήνα δεν παρατηρήθηκε εμφανής εξωφυτικού τύπου βλάβη. Η κήλη λευκής γραμμής επανελέγχεται χωρίς εμφανή σημάδια περίσφιξης.
Συμπέρασμα
Αύξηση των διαστάσεων των οζωδών ενισχυμένων πυκνωτικών εστιών στο οπίσθιο κοιλιακό τοίχωμα αριστερά, στο επίπεδο της νεφρεκτομής και γύρω από τον κάτω πόλο του σπλήνα είναι ύποπτη για τοπική υποτροπή.

Stress hormones in North Atlantic right whales fell during a lull in shipping activity following the September 11, 2001 attacks on the World Trade Center and Pentagon. The finding suggests that ship noise may do more than annoy the marine mammals.
Observations in the wild have already shown that noisy ships can change whale behavior. Engines and propellers give off low-frequency sounds that carry well underwater and overlap with the frequencies whales use to chitchat. To be heard in a din, the social animals increase the volume, pitch or length of their calls. Often they turn away from sudden sounds or avoid loud waters altogether.
“This is the first evidence that whales have a physiological response to noise,” says veterinarian Rosalind Rolland of the New England Aquarium in Boston. Her team’s findings appear online February 8 in Proceedings of the Royal Society B: Biological Sciences.
To see if these responses to noise stem from anxiety, Rolland’s team began measuring stress hormones in the feces of North Atlantic right whales in Canada’s Bay of Fundy in 2001. During the summer, these endangered whales feed and breed next to busy shipping lanes in this narrow body of water.
The attacks of September 11 interrupted this experiment. With fewer large ships moving in and out, the bay suddenly quieted, especially in the low frequencies thought to bother whales.
This relative peace seems to have relaxed the whales, says Rolland. Stress hormones in her team’s 2001 samples were lower in the months after September 11 than in the months before. No similar decline was seen in four subsequent years of monitoring, suggesting that other factors in the environment weren’t to blame.
