يتسبب مرض الروماتويد في تدمير 50 % من المفاصل خلال أول عامين من الإصابة.. من هنا جاءت التوصيات الأوروبية والأمريكية الحديثة بتغيير مفاهيم تشخيص ومراحل المرض, وذلك بإعطاء المريض علاجه خلال الـ3 أشهر الأولي من الإصابة.
قبل حدوث التآكل العظمي للمفاصل, مع تثبيط المرض نهائيا وليس جزئيا لتسببه في زيادة أمراض القلب والشرايين مقارنة بالأصحاء.. هذه النتائج ناقشها المؤتمر الدولي الـ16 للجمعية المصرية للروماتيزم والتأهيل.
ويقول الدكتور حسن الشهالي أستاذ الروماتيزم والتأهيل بطب قناة السويس ورئيس الجمعية والمؤتمر انه ناقش الاستراتيجيات الحديثة لعلاج الذئبة الحمراء ومنها استخدام الرنين المغناطيسي لتشخيص إصابات القلب والرئة, والعلاج بمثبطات الخلايا الليمفاوية بي, وعلاج هشاشة العظام بهرمونات الغدة الجاردرقية لاستعادة نمو وكثافة العظام, بالإضافة للحقن الموضعي لتحسين حالة المفاصل والغضاريف في مرضي خشونة المفاصل, وتحسين حركة الأطفال المعاقين من خلال المساعدات والحقن الموضعي بمادة بوتكس.
ويصيب التهاب العضلات المزمن الأطفال في سن5 إلي15 ومتوسطي العمر بين30-50 عاما, وتظهر أعراضه في ضعف عضلي بالأكتاف والحوض وصعوبة في رفع الذراعين وصعود السلم, وقد يصاحب المرض طفح جلدي والتهاب مفصلي, والتهاب بالرئتين, والتهاب بعضلات القلب, وطبقا للدكتور الشهالي فإن التشخيص يكون بتحليل عينات أنسجة العضلات بالفحص الكيميائي وتحليل الأجسام المضادة بالدم, والتي تختلف حسب أعراض المرض الإكلينيكية, وتعد أشعة الرنين المغناطيسي احدي وسائل التشخيص, وجهاز رسم العضلات الذي يميز بين الضعف العضلي نتيجة مرض الالتهاب العضلي, وإصابة الأعصاب, ويكون العلاج المكثف بالكورتيزون والعلاج الطبيعي والوظيفي لأجزاء الجسم المصابة, مع الوقاية من أشعة الشمس, وتجنب زرع الكولاجين والسيلكون.
وتناول الدكتور محمود الطيب أستاذ الروماتيزم والتأهيل بطب عين شمس ورئيس جامعة حلوان وضع خطوط استرشادية لعلاج الروماتويد بين مصر وهولندا وايطاليا وألمانيا توصي باستخدام مثبطات المناعة مباشرة في المراحل الأولية للمرض, واللجوء للعلاج البيولوجي في الحالات المتأخرة, مع تأكيد تقدم طرق الكشف المبكر والتشخيص بصورة تتيح أعطاء العلاج قبل إصابة المفاصل والغضاريف بأضرار. وعلي عكس الشائع.. من صعوبة استعادة المفصل لطبيعته بعد الإصابة بالخشونة, تناول الدكتور محمد علي علوي أستاذ الروماتيزم والتأهيل بطب عين شمس الجديد في علاج خشونة المفاصل في مراحل الإصابة الأولية باستخدام العلاجات الحديثة التي تعمل علي مستوي الخلية والتي تتيح استعادة المفصل لحيويته, وأكد أهمية تجنب مسببات خشونة المفاصل كزيادة الوزن وتعلم طرق الجلوس والوقوف الصحيحة وممارسة التمارين الرياضية بانتظام وعند حدوث الخشونة يجب اتباع العلاج الدوائي مبكرا للتحكم في تحسن المفصل. ويعتبر الروماتويد من عوامل الخطورة للإصابة بأمراض القلب, كما ترتفع نسبة الوفاة بمرضي الروماتويد نتيجة أمراض القلب إلي 50 %, وكما تقول الدكتورة سامية عبد المنعم أستاذ الروماتيزم والتأهيل بطب بنها أن مرضي الروماتويد ترتفع لديهم العوامل الالتهابية والحمض الاميني الهوموسيستين المسببين للالتهاب ويكونون عرضه لتصلب الشرايين التاجية, وموت عضلة القلب وهبوطها, كما أن بعض علاجات الروماتويد ترتبط بأمراض القلب كالكورتيزون والأدوية البيولوجية, ويوصي بعلاج الروماتويد بطريقة سليمة, وأتباع الأسلوب المتبع مع مرضي القلب كالامتناع عن التدخين والتغذية الصحية والرياضة وتظهر الخطوط الإرشادية للكلية الأمريكية والرابطة الأوروبية لتشخيص الروماتويد تقدما كبيرا كما يقول الدكتور ياسر الميداني أستاذ الروماتيزم بانجلترا في التشخيص اتاح اكتشاف المرض في مراحله المبكرة, ويكون التشخيص بالموجات الصوتية والرنين المغناطيسي علي المفصل لتحديد الالتهابات للحفاظ علي المفصل دون تشوهات, بجانب تقدم مثبطات التهابات المفاصل ووجود4 أجيال حديثة من العلاج البيولوجي.
Translation - English Rheumatoid Arthritis Early Detection, Diagnosis Techniques another Step Forward
Nearly 50 percent of rheumatoid arthritis patient's joints are destroyed only two years after the infection is detected, something which urged Europe and the US to change the disease detection and diagnosis concepts, by giving the patient rheumatoid arthritis medication during the first 3 months of infection.
By so doing, doctors want to inhibit the disease totally and not partially before the patient's joints experience bone erosion.
Rheumatoid arthritis is blamed for the rise in cardiac and artery diseases in its patients compared to healthy people.
These findings have been discussed during the 16th international conference of the Egyptian Society for Rheumatology and Rehabilitation.
Society Chairman, Hassan Shahali, rheumatology and rehabilitation professor at Suez Canal University's School of Medicine, said the conference discussed most recent technologies for lupus erythematosus treatment such as using magnetic resonance in diagnosing heart and lung infections, or lymphatic cells depressants B, as well as the treatment of bone loss with parathyroid gland hormones to help bones restore density.
The conference also discussed using local injection as a means to improve joints and cartilage conditions in patients of joint stiffness and to improve the movement of disabled children.
Chronic myositis (muscle inflammation) affects children between ages 5 and 15 and middle-aged people between 30 and 50. Its symptoms include muscle weakness in the shoulders and pelvis and difficulty in lifting the arms and climbing the stairs. Skin eruptions and joint arthritis may also appear, as well as pneumonia and heart muscle inflammation.
Diagnosis is performed by chemically analyzing samples from muscle tissues and by analyzing antibodies in the blood, which differ according to the disease clinical symptoms.
Magnetic resonance ray is one great diagnosis technique, as well as electromyograph (An instrument used in the diagnosis of neuromuscular disorders).
Cortisone is used in the treatment, in addition to physical and physiological therapy, while the patients must not be exposed to sun and must not get any collagen or silicone into their body.
Rheumatology and rehabilitation professor at Ain Shams University's School of Medicine and Helwan University President, Mahmoud al Tayyeb, suggested implementing guidelines for rheumatoid arthritis patients between Egypt, Holland, Italy and Germany, to recommend using immunity depressants in the early stages of the disease and biological medicine in the late cases.
It is commonly believed that prescriptions can never help joints restore their normal condition. However, Dr. Mohamed Ali Alawi, rheumatology and rehabilitation professor at Ain Shams University tackled what's new in joint stiffness treatment techniques, saying the disease can now be treated in its early stages using modern medicines that work at the cellular level and help joints work well again.
Being overweight is one factor blamed for joint stiffness. People need to learn proper ways of sitting down and standing up and to exercise regularly.
Prescriptions are better used in the early stages of the disease in order to help the joints improve before they worsen.
Doctors are sure rheumatoid arthritis causes heart disease, blaming it for the death of 50 percent of its patients.
Dr. Samia Abdel Monem, rheumatology and rehabilitation professor at Benha University said that rheumatoid arthritis patients suffer high inflammation susceptibility and the amino acid homocysteine that causes inflammation. They are venerable to coronary arthrosclerosis and heart muscle death.
Some rheumatoid arthritis prescriptions relate to heart diseases like cortisone and biological drugs.
Like people suffering from heart disease, rheumatoid patients are advised to quit smoking, eat healthy food and exercise.
