Source text - Japanese Pretibial myxedema（以下PTM）は、下腿伸側の皮膚真皮に酸性ムコ多糖が限局性に沈着する代謝性疾患である。PTMは欧米ではバセドウ病の約5%、本邦では0.5～1%に合併すると報告されている（1 ）。本邦の報告においては、PTMの男女比は1:1.8で女性に多く、男性は40代、女性は50代にピークを有する（1）。甲状腺機能は約90%がhyperthyroidで生じ、 10%がeuthyroidまたはhypothyroidの状態でも生じており(2)(3)(4)、必ずしも甲状腺機能の異常の程度とは相関しない。またopthalmopathyとして眼球突出（Exophthalmus）を、dermopathyとしてPretibial myxedemaを、Osteoarthropathyとしてばち指を伴うことがあり、この3症候がそろった症例はEMO症候群と呼ばれている（5）（6）。今回、L-T4の内服中断による潜在性甲状腺機能低下症の増悪に伴い顕在化し、L-T4 内服の再開、およびステロイド外用剤塗布にて皮膚所見の明らかな消退を認めたPTMの一例を経験したので報告する。
Translation - Spanish El mixedema pretibial (en adelante, MP) es una afección metabólica provocada por la acumulación localizada de mucopolisacáridos ácidos en la región pretibial. Hay informes que indican que el MP se presenta como una complicación de la enfermedad de Graves Basedow, con una incidencia de aproximadamente el 5% en Europa y del 0.5-1% en Japón（1）. Según los informes japoneses, la afección tiene una incidencia superior en las mujeres, con una proporción de 1:1,8. La incidencia pico en hombres se observa entre los 40 y los 50 años y en las mujeres entre los 50 y los 60(1). Aunque la función tiroidea no esté directamente relacionada con la enfermedad, aproximadamente el 90% de las veces se presenta en casos de hipertiroidismo, y el otro 10%, en situación de eutiroidismo o en casos de hipotiroidismo(2)(3)(4). Los casos en los que aparece la tríada de síntomas compuesta por la oftalmopatía exoftalmía, la dermopatía MP y la osteoartropatía acropaquia designan el llamado síndrome EMO(5)(6). En este informe se pretende evidenciar que la supresión de la LT4 comporta un empeoramiento de los casos de hipotiroidismo latente y, al mismo tiempo, presentar un ejemplo de clara mejoría en un caso de MP al retomar el tratamiento con LT4 acompañándolo del uso tópico de crema de esteroides.
