I think I figured them all out, except one 1. צ. הור' is ציון הוראה, which is the grade given by the teacher = Teacher's Grade
2. צ. מש' is ציון משימות, which is the grade achieved in tests etc. =Assignments' Grade
3. צ. תק' is ציון תקופתי - the final, actual, weighted grade for the period/semester.

I will put them in an answer, so you don't need repost them separately...

(We can try to figure out the ס.ת. separately, if you provide more context, or more examples)

A. As per Kudoz rules the terms should be separated into several entries, not just for credits (though that's important too), but so that each entry can be later used separately as a term, searched, or leveraged in other ways.

B. Personally I'm pretty sure I figured out at least צ. הור' and צ. מש' (Teacher's Grade and Assignments' Grade, respectively)

C. It would help if you paste a larger sample of the text, for context.

I am quite sure that's what it stands for, since this is indeed the method used to calculate the grades in Israeli schools:

1. צ. מש' is ציון משימות, which is the grade achieved in tests & other assignments.
I'd use Assignments' Grade or Tests Grade
2. צ. הור' is ציון הוראה, which is the grade given by the teacher.
I'd use Teacher's Grade
3. צ. תק' is ציון תקופתי - the final grade for the period/semester, calculated as the (sometimes weighted) average of the above two.
I'd use Period Grade

You could abbreviate:

Tst Gr., Tch Gr.Prd Gr.
