Source text - English With the unit at 6.5 percent power in preparation for main turbine startup, an inadvertent overfeeding of the steam generators resulted in a main turbine trip. The reactor operator began a rapid control rod insertion to reduce reactor power to less than 3 percent. In addition, the control room supervisor directed the steam dumps to be manually opened. Reactor coolant system temperature was reduced. Subsequently, in response to a greater than expected decrease in power, the control rods were withdrawn by the reactor operator to raise reactor power from 0.2 percent to 2 percent. Temperature was restored to the no-load condition.
Important Factors:
The reactor operator rapidly inserted control rods without knowing the resultant end state.
The subsequent withdrawal of control rods occurred while positive reactivity was also being added by reactor coolant temperature reductions.
Manual operation of the steam dumps exacerbated the transient.
Causal Factor:
Reactivity management requirements were unclear regarding withdrawal of control rods during transient conditions.

Source text - Spanish Infarto Cerebral es la muerte de alguna zona del tejido encefálico y que generalmente se debe a un problema por embolización arterial de material trombótico, y que su origen puede ser diverso. En el 20% de los casos es de origen cardioembólico, ya que el material trombótico se forma en la pared auricular o ventricular o en las válvulas de la mitad izquierda del corazón. Cuando el trombo se fragmenta, suele aparecer momentáneamente un TIA. Pero cuando la oclusión arterial se prolonga se origina EVC.
Generalmente son de inicio repentino e inmediatamente se alcanza la deficiencia neurológica máxima. Es importante hacer el diagnóstico diferencial con la hemorragia cerebral o intracraneal franca, donde existe efecto de “masa” por la hemorragia, lo que provoca deterioro de la función neurológica, que traduce a su vez, el tiempo transcurrido entre el diagnóstico y el daño neurológico secundario (pronóstico para la vida funcional7 futura de la persona que lo tiene) al efecto de “masa”.
Translation - English Brain infarction is the death of an area of encephalic tissue, generally due to a thrombotic material arterial embolization problem and of diverse origin. In 20% of cases its origin is cardioembolic, given that the thrombotic material is formed on the auricular or ventricular walls or on the left side of the heart valves. When the thrombus is fragmented, a Transient Ischemic Attack (TIA) often appears momentarily, but a Cerebrovascular Disease (CVD) takes place when arterial occlusion is prolonged.
They often onset suddenly and the maximum neurological deficiency is reached immediately. It is important to make the differential diagnosis with marked brain or intracranial hemorrhage, where there is a "mass" effect due to the hemorrhage, which provokes deterioration of the neurologic function; the time elapsed between the diagnosis and the secondary neurological damage (prognosis for the patient’s future functional life) translates into the "mass" effect.
