I am not sure what this term may refer to in contexts such as the following:

Does it refer to expenses incurred for patients that have received the study drug/completed the trial as opposed to patients who have dropped out?

I think the most literal translation works fine here. Implementation has the sense of putting into effect or setting up. This works well considering it's the charge for subjects up to initial study drug dosing.

Per-subject costs
I couldn't find an exact translation for the term. I would use per-study, per-subject, and per-enrollment costs for each invoice method listed on the slide.
