one of the documents that the seaman needs to provide while applying for Crew Visa at the U.S.Embassy. The translator wrote: "potvrda o neprekinutom otpustu", but i am not sure this is a correct translation, as this term in English means:
This document certifies that the person holding this is a seaman as per The International Convention on Standards of Training, Certification and Watch keeping for Seafarers (STCW), 1978, as amended 1995. Every seafarer must carry this document while on board, which is also an official and legal record of his sea experience. The master of the vessel signs the document each time a seaman is signed off from the vessel certifying his experience on board.

As for the answer that you found, no references were provided to back it up so there is no way to check. Unfortunately on ProZ.com, the correct answer is not necessarily accepted by the asker and entered into the glossary.
Note from asker:
the text originally says: "A seaman's book or continuous discharge certificate", so this should be something different from a seaman`s book. It is smth more like a Seaman ID card.
