ДТК

Bulgarian to English Medical Medical: Health Care
I'm duing some immunization reports for the Toronto School Board, and one of the cards seems to be in Bulgarian rthaer than Russian. There's only two entries I can't figure out, one of them is ДТК (вакцинация/ревакцинация) and the other is an abbreviaiton that looks like "изв."
Anybody familiar with this sort of thing?
(The other vaccinations listed are BCG, Polio, Trimovax and Hep B)

2. "Изв." may stand for "извлечение"(extract) in an academic transcript
